If LOCALLY employed:

  • Latest Income Tax Return (ITR) or Certificate of Creditable Tax Withheld (W2) at Source for at least three (3) years
  • Photocopy of 2 Government Issued ID's with 3 specimen signature
  • Latest Proof of Billing
  • BIR Form 2316
  • Certificate of Employment and Salary; payslip for the past three (3) months

If employed ABROAD :

  • Latest 3 months proof of remittance, or
  • Latest 3 months payslip, or
  • Cert of Employment or Employment Contract duly authenticated by Phil Consulate Office, or
  • For seamen, latest Crew Contract and employment history or Crew Contract and Exit Pass from POEA.
  • SPA notarized and with Philippine consular seal (if the signatory to the transaction is other than the buyer.  Note: 

If in business or self-employed:

  • Income Tax Return and Audited Financial Statement for three (3) years
  • In-House Financial Statements for at least three (3) years
  • Certificate of Business Registration (SEC or DTI)
  • Articles of Incorporation / Partnership and By-Laws
  • Bank Statements (latest 3 months)
  • Business Background / Company Profile
  • List of major suppliers and customers (at least 3) (pls. indicate telephone number and contact person) or List of Affiliated Hospitals/Clinics (for doctors)
  • Lease Contracts and copy of title (for income derived from rental)


Additional requirements:

  • Marriage Contract (if applicable)
  • Bank Statements / Certification
  • Titles to properties owned but not offered as collateral (if any)
  • Lease contracts of income generating properties (if any)
  • OR/CR of vehicles owned (if any)
  • Stock Certificates (if any)

Information on collateral property:

  • Photocopy of the title
  • Certified lot plan and location / vicinity map
  • If loan is for construction / renovation: Bill of materials, Building plans and
  • specifications
  • If loan is for refinancing: Statement of account from the creditor / mortgagor
  • Master Deed of Declaration of Restrictions (for condominiums)

Types of Acceptable Visa for Foreigners

  • Quota or Non-Quota Immigrant Visa:
  • Alien spouse of a Filipino citizen (Sec 13A Visa)

-  Natural-born Filipinos married to an Alien (Sec 13D Visa)
-  Returning Resident (Section 13E Visa)
-  Natural-born Filipinos naturalized in foreign country (Sec 13G Visa)

  • Photocopy of Special Retiree's Resident Visa (SRRV) or Quota/Preference Immigrant Visa
  • Permanent Resident Status and working Visa granted by Subic Bay Metropolitan Authority (SBMA)
  • Working Visa issued to Embassy Officials

For Refinancing or Loan Take-out

  • Statement of account and official receipts for the last 3 months
